In Vitro Fertilization (Surrogacy)  is a form of assisted pregnancy for those who may not be able to conceive naturally. In the  monthly ovulation cycle, one of the ovaries releases an egg and after ovulation the egg enters the fallopian tube. The woman likely gets pregnant if she has sex just before, during or right after ovulation. During this time, a sperm cell from a man's semen  is able to fertilize the egg while it is inside the fallopian tube. As it travels through the fallopian tube the  fertilized egg or the embryo  possesses its outer shell  that nourishes it  while an ovary. It then enters the uterus and loses the protective outer layer called zona pellucida. In a process called zone hatching, the embryo gets implanted into the lining of the uterus. This makes a permanent connection between the women and the embryo and forms an organ called placenta which allows oxygen and nutrients from the women to the embryo and the pregnancy continues. Infertility, single parenthood etc are the conditions where Surrogacy is implemented. Before Surrogacy, the women must undergo a medication process so that more than one egg is grown and matured. A semen sample is required from the man so that the healthiest sperm can be collected  in order to enhance the chance of pregnancy. In case, the male partner is sterile or the woman   doesn’t have a partner  a donor semen is used. Surrogacy involves three main processes:

Follicle Aspiration : Here, the eggs are harvested from the  ovaries using suction devices connected to a needle. The needle penetrates vaginas and the ovary to collect several eggs at a time. 

Fertilization : In this step, the eggs are placed in a petri dish and artificially inseminated using the enriched and processed sperm, from the semen sample collected. Eggs are either mixed with sperm or a single sperm is injected into the egg. The second process is called Intracytoplasmic Sperm Injection. The embryos are then monitored for 3-5 days in the laboratory  and the lab may create Zona Pellucida around some of the embryos. This assisted hatching helps embryos to get implanted to the uterus. 

Embryo transfer: The fertilized  eggs are  transferred to the women’s uterus. Hormonal therapy is done to help embryo’s  to get implanted. Some embryos are transferred right away and some are frozen for future use. A tool called ‘speculum’ is inserted into the vagina so  the cervix is clearly visible. Fluid  containing embryos is taken into a syringe. The syringe is attached to a long catheter which is then inserted into the uterus via cervix. The embryos are then injected inside the uterus of the women which will lead to pregnancy.

Reason for Failed Surrogacy

  • Age is the biggest factor that can lower down the success rate of Surrogacy  as the younger  the woman is, the quality of embryo increases and the ovarian response may be less effective in older women. The sperm quality in men decreases if the man grows older . Therefore, younger women have higher chances of success in Surrogacy treatment. The success rate starts decreasing after 35 years

  • Failure of Surrogacy can also prevail for genetic reasons. So, preimplantation genetic testing (PGT) is conducted to help identify the  issues which might lead to higher success rate in Surrogacy.

  •   The frozen or older eggs have insufficient spindle apparatus which leads to abnormal embryo development.
