The arrival of the two-child era has given many women the opportunity to have another baby, and cesarean section is a topic that every mom-to-be has not been around. Natural birth is best, but there are always breaks, such as when the baby is breech,剖腹產寶寶 is in the breech position, weighs more than 8 pounds, and the natural birth didn't go well, etc. It's important to choose a C-section for these times. Let's talk about cesarean delivery today.

What is the incision for a cesarean?

There are two types of incisions for a clinical cesarean section: horizontal and vertical, which are chosen based on the individual circumstances of each mother-to-be. The aim is to minimize bleeding and tissue damage, promote healing and ensure the safety of the mother and baby.

The most commonly used incision is the horizontal incision, which is a horizontal cut in the lower part of the abdomen. The incision is relatively low, and the length of the incision needs to be fully estimated according to the width of the fetal head in our country, usually 10 cm. The horizontal incision is also known as the bikini incision, which means that there is no time for healing after the scars are invisible in a bikini, which is aesthetically pleasing.

The longitudinal incision is about 10 centimeters long from the navel to the top of the pubic bone, and the scar is not very pretty as it can be clearly seen when wearing clothes that expose the abdomen.

Although the horizontal incision is now a popular trend, expectant mothers do not necessarily have to choose the incision they want. In the event of an emergency, both the mother and the baby are in danger, and the sooner the baby comes out, the better. In this case, a vertical incision is preferred, and aesthetics should be put aside for the time being.

What is the best incision for a second cesarean section?

Nowadays, more and more mothers are having their second child. For a second cesarean section, if nothing else, they usually choose to cut the original scar tissue through the original scar incision and then suture it back together after surgery. Since the original scar tissue is not as flexible and fibrous as the original scar tissue, the growth of new tissue after excision is beneficial to wound healing.

As with the first cesarean section, if there is an emergency situation that threatens the life of the mother and baby, such as fetal asphyxia or placental abnormality, regardless of whether the first cesarean section was a transverse incision or a longitudinal incision, a longitudinal incision can be chosen in order to save the life of the mother and baby first and foremost.

Do I have to remove the stitches from the incision?

Nowadays, transverse incisions are usually made with intradermal sutures using absorbable threads, which do not need to be removed, while some do.

Longitudinal incisions are usually made with silk sutures, each stitch is sewn individually and needs to be removed 7 days after surgery.

Don't be afraid of the itchy feeling before the suture is removed, just like being bitten by ants, it is not very painful and can be tolerated.

Will a second cesarean section be particularly painful?

Many mothers who are having their second child feel as though having a second cesarean section is more painful than the first, and this is indeed a possibility. Existing socio-medical education studies have found that a second cesarean section can cause secondary damage by stimulating damaged skin tissues, making the mother-to-be more sensitive to pain; surgical stimulation of tissues and intercellular production of inflammatory painkillers such as bradykinin, histamine, and potassium ions, causing pain in the mother-to-be. Moreover, after the birth of a child, the uterus will produce a strong and persistent contraction, which can enhance the pain of contraction, and if the mother is sensitive to the contraction of the uterus, the pain will be more obvious.

So what can we do to alleviate this pain? First of all, the mother should understand the cause of the pain and eliminate her worries and fears. Secondly, we should do a good job of home care and help the mother to get up and move around early. Six hours after the surgery, according to the mother's preference, she should choose foods that are high in protein, easy to digest, minerals and vitamins, and put on some soothing music to distract her attention. Of course, pain is serious and there are painless birthing techniques that can help, such as using a pain pump or pain medication, which are also great. Don't force the pain, it may backfire.
